  • Clause: You are responsible for providing accurate and complete information about your medical history, current health conditions, medications, and lifestyle to Physique SI. Failure to do so may impact the safety and effectiveness of the services provided.

  • Clause: You agree to follow all instructions and recommendations provided by Physique SI medical professionals regarding treatment plans, medication usage, follow-up appointments, and lifestyle modifications.

4. Medical Consultation and Informed Consent

  • Clause: Prior to commencing any TRT or peptide therapy at Physique SI, you will undergo a medical consultation with a qualified healthcare professional. This consultation is essential to determine your suitability for treatment and to discuss potential benefits, risks, and alternatives.

  • Clause: You understand that TRT and peptide therapies carry potential risks and side effects. You will be provided with detailed information about these risks and benefits during your consultation and will be required to provide informed consent prior to commencing any treatment.

  • Clause: You acknowledge that results from TRT and peptide therapies may vary, and Physique SI cannot guarantee specific outcomes.

5. Payment and Fees

  • Clause: You agree to pay all fees associated with services provided by Physique SI as outlined in our fee schedule, which may be updated from time to time. Fees may include consultation fees, treatment fees, medication costs, laboratory testing fees, and other applicable charges.

To the fullest extent permitted by applicable law, Physique SI disclaims all warranties, express or implied, including but not limited to warranties of merchantability, fitness for a particular purpose, and non-infringement.

10. Limitation of Liability
